This health insurance plan is for those who are leaving or have left a group benefits plan recently. Regardless of which insurer you were insured with.

Your Manulife quote confirmation number if you have had an online quote done.
OHIP Health Card numbers for everyone applying for coverage.
Date of Birth for each person being covered.
Name of Insurance Company, Group Policy Number and your Certificate Number of the group benefits plan you are leaving.
Credit Card Information or bank information for pre-authorized payment plans.
