For new applicants who have not done a quote – on the far right  of the apply online page is a link (hard to see) ‘complete application’
For applicants who have done a quote or started an application –  press ‘Continue Application’  on the apply online page, and then enter your quote number where it says application number.  If you do not have a quote number – click on the ‘application finder’ button of the apply online page.

 

 

What you will need with you when applying online:

Your quote confirmation number if you have had an online quote done.
OHIP Health Card numbers for everyone applying for coverage.
Date of Birth for each person being covered.
Name of Insurance Company, Group Policy Number and your Certificate Number of the group benefits plan you are leaving.

Credit Card Information or bank information for pre-authorized payment plans.
